Complete the Online Physical Registration<\/strong><\/p>\n<p>All athletic paperwork must be submitted through the new online platform:<\/p>\n<p><a href=\"https:\/\/students.arbitersports.com\/liberty-high-school-pa\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=https:\/\/students.arbitersports.com\/liberty-high-school-pa&amp;source=gmail&amp;ust=1784219617196000&amp;usg=AOvVaw1eKZiRzBW3Pk92NsdRPcfD\"><strong>https:\/\/students.<wbr \/>arbitersports.com\/liberty-<wbr \/>high-school-pa<\/strong><\/a><\/p>\n<ul>\n<li>Create a family account or log in to your existing account.<\/li>\n<li>Complete all required information for your child.<\/li>\n<li><strong>Sections 5 &amp; 6<\/strong>\u00a0of the PIAA Physical Form must be completed and signed by a licensed physician.<\/li>\n<li>Upload the completed\u00a0<a href=\"https:\/\/drive.google.com\/file\/d\/1B3q66CHDn8pZsiVb62YW2ZcR1XciLcgh\/view?usp=sharing\"><strong>Sections 5 &amp; 6<\/strong><\/a>\u00a0form to the online portal.<\/li>\n<\/ul>\n<p>If you have any questions regarding the physical paperwork or online registration, please contact\u00a0<strong>Liberty High School Athletic Trainer, Daulton Lacava<\/strong>, at\u00a0<a href=\"mailto:daulton.lacava@sluhn.org\" target=\"_blank\" rel=\"noopener\"><strong>daulton.lacava@sluhn.org<\/strong><\/a>.<\/p>\n<p><strong>2.
