The Bethlehem Area School District is eligible to receive federal funds under the Elementary and Secondary Act (as amended by the Every Student Succeeds Act)– Impact Aid Section 7003. In order to receive these funds, it is necessary for us to identify students who have a parent/guardian on active duty in the uniformed services of the United States on December 6, 2021, which is the survey date for our application.  This includes Army, Navy, Marine Corp, Air Force, Coast Guard, National Oceanic and Atmospheric Administration, and U.S. Public Health Service personnel.  If you have a child in your household who has a parent/guardian on active duty on December 6, 2021, please complete the information below and return this form to your child’s school by Friday, December 10, 2021.
