Student Registration
Students will need to be registered at the resident school that services your residence (home address). If you do not know which school your child should attend, please call the Child Accounting Department (610-861-0500, extensions 60272, 60273, or 60274) to determine which school services your home address. Grade 6 – PA State Mandated Physical Exam
Sixth Grade Parents: All 6th graders are required to have a physical exam. If your child has had a recent exam, please have their provider complete the Physical Exam form and return it to the Nurse’s Office. If they have not yet had a physical exam, please schedule one as soon as possible.
Your child will need to receive the mandated 7th grade vaccines (TDAP, MCV4) prior to the start date of their 7th grade school year or they will not be able to start school. Physical exam/vaccine appointments fill up quickly in the summer, scheduling an appointment early will ensure they are able to receive their vaccines in plenty of time! Please notify the Nurse’s Office of the date/time/provider for the physical exam, so we can note it on their record.

Seventh Grade Parents: All 7th graders are required to have a dental exam. If your child has had a recent exam, please have their provider complete the Dental Exam form and return it to the Nurse’s Office. If they have not yet had a dental exam, please schedule one as soon as possible.
