Northeast MS Athletic Physical Information – Fall Sports
Fall sports will begin in approximately one month! To ensure your child is eligible to participate on the first day of practice or tryouts, please complete the required athletic paperwork outlined below.
1. Complete the Online Physical Registration
All athletic paperwork must be submitted through the new online platform:

- Create a family account or log in to your existing account.
- Complete all required information for your child.
- Sections 5 & 6 of the PIAA Physical Form must be completed and signed by a licensed physician.
- Upload the completed Sections 5 & 6 form to the online portal.

2. Submission Deadlines
All required athletic paperwork must be completed and submitted no later than one week before your child’s first official practice or tryout.
- Football: August 10
- Cheer, Field Hockey, Volleyball, and Cross Country: August 17
